We’ll establish the support you need to attain your goals and guide you through the steps to execute your plan.
Our NDIS Support Coordination team members help you navigate unforeseen challenges or changes and are also highly flexible. We understand that your needs may evolve over time, and we work closely with you and your support network to make sure your NDIS plan and personal goals are achieved. We always ensure participants' needs and goals remain committed to help you use your chosen support and are top priorities.
What is support coordination, and how does a support coordinator help?
Support Coordination is a programme that helps individuals with disabilities access and understand their NDIS plan.
You’ll be allocated a support coordination provider, who will work closely with you to reach your goals, helping you build knowledge and skills so that you feel more confident.
Over time, you may gain the confidence and skills to manage your NDIS plan independently.
Your support coordinator will:
I. Help you access and understand your NDIS plan.
ii. Let you know what services are most suitable for your needs and what you can access
Iii. Assist you in choosing services that fit within your budget.
Iv. Negotiate the costs for the services you choose.
V. Support you in building confidence to manage your NDIS plan your way.
Vi. Help you access the services you have chosen.
What is included in support coordination under NDIS?

Navigating the NDIS: NDIS will help you understand your plan and show you how to get the best out of it.

Goal Tracking and Reporting: Professionals will help you keep track of how well you're doing in attaining your goals, so you can see how much you've accomplished during your time with NDIS.
Support Implementation: We’ll monitor your progress toward your goals and help you get ready for future NDIA planning meetings.
Confidence and Capacity: We’ll help you develop your skills and abilities, empowering you to become more independent and confident.
OUR PHILOSOPHY
Our goal is to provide quality care that assists each individual with their physical, emotional, and daily living needs. We aim to address the needs of each individual, and to encourage and support them in enjoying an independent lifestyle.
We are proud of our reputation for providing a caring and harmonious family environment. Our staff members have been with us for many years and are all dedicated to caring for our participants. Our owners and staff gladly go beyond their duties to ensure our clients are happy and well cared for, and that the atmosphere is always positive. All staff hold certificates for training appropriate to their position.
It is our aim for participants to remain independent, and we encourage all clients to take care of themselves as much as possible. However, extra services can be arranged where help is needed. These services include laundry, changing sheets, providing extra linens, and room cleaning. These extra services will incur additional charges if not included in NDIS plan.
We offer assistance with medications onsite for breakfast, lunch, and dinner. We use a blister pack system and store medications in a secure area in a locked trolley. Assistance with medication is given as per the doctor’s instructions. Records of medication taken are kept on a signing sheet.
